Positions Available: Site Coordinator
Job Status: Full Time
Locations: CMS School Site
Primary Function: Identify and assess the needs of at-risk students through a case management model, coordinating activities and services that guide students toward fulfillment of their potential for academic, physical, and social-emotional growth. Work with students and families to monitor attendance and encourage and prepare students for promotion to the next grade and beyond.
Major Areas of Responsibility:
- Provide case management to identified group of students, ensuring social, emotional, medical, and educational needs are addressed
- Build one-on-one relationships with students and families
- Coordinate staffing aimed at assessing needs, setting goals, and strategizing around interventions, linking school professionals, parents, and appropriate agency/community personnel
- Work with school system to monitor attendance, academics and behavior; taking appropriate action
- Coordinate staffing aimed at assessing needs, setting goals, and strategizing around interventions, linking school professionals, parents, and appropriate agency/community personnel; coordinate and/or provide student enrichment activities, based on age and level
- Coordinate and participate in student-training efforts to enhance life skills using community resources/support
- Recruit tutors, mentors, engage parents, and participate on school teams
- Provide partnerships and on-site coordination of human services to the school site that address the needs of students enrolled in program
- Complete appropriate school/CIS documentation to capture services provided, as well as other necessary documents and reports
- Coordinate post-secondary/college access services and programming for enrolled students
Job Requirements:
- Bachelor’s degree (Masters preferred) in human services, social work, counseling, education, or public administration field
- Minimum of 3 years of working experience working with teens in a professional capacity in the areas of education, case management, or social work; knowledge of school systems preferred.
- Computer skills utilizing Microsoft Office
- Strong oral and written communication skills with professional presentation skills (both large and small groups)
- Self-motivated with the ability to make independent decisions, engage with students and parents
- Knowledge of Youth Behavioral Issues/Community programs and resources available preferred
- A valid NC driver’s license, with an acceptable driving record is required
