Position Available: Case Manager

Primary Function: This position will provide: management, assessment, and evaluation services to 20-25 teen parents and their children, age birth -5; conduct home visits and support to clients; services to teen parents to assist them in high school completion, healthcare, and immunization; generate documentation, reports and other necessary correspondences; collaborate with other youth and human service agencies, staff and services teams in the community. This position reports directly to the Director, Teen Services for Safe Journey/APP.
Major Areas of Responsibility:
Ø Coordinate the process of identifying, selecting and enrolling targeted at-risk teen parents
Ø Support and provide services to target parents that will assist them to stay in school and complete their high school education
Ø Educate teen parents about effective social, health, and employment support systems
Ø Coordinate and assist program participants with health care and immunization needs
Ø Facilitate group/individual parenting sessions, in order to educate participants about age appropriate issues related to child development.
Ø Collaborate with other youth/human service agencies, staff and service teams in the community to provide needed services to clients, as identified.
Ø Provide counseling to teen parents and their families to improve decision making and problem solving skills
Ø Assist with overall program development, outcome measures and budget accountability
Ø Maintain knowledge of available community resources
Ø Consult with school guidance counselors and other collaborative partners
Ø Participate and coordinate student-training efforts to enhance life skills using community resources/support
Ø Document and maintain directory of clients served and their corresponding data for program management and reporting
Ø Generate case notes, court reports and other necessary correspondences monthly
Ø Maintain records related to child care subsidy referrals provided through Smart Start in collaboration with Child Care Resources, Inc.
Ø Provide monthly program reports to Program Director
Job Requirements:
Ø Bachelors degree in human services, social work, counseling, education, or public administration
Ø Minimum of 1-2 years successfully working with adolescents and evidence of working within a collaborative service team
Ø Ability to work some evenings
Ø Strong interpersonal skills to work successfully one-on-one and in team settings
Ø Working computer skills to include utilization of Microsoft Office
Ø Exceptional oral and written communication skills
Ø Self-motivates with the ability to make independent decisions
Ø Effective analytical, planning, and problem solving skills
Ø Knowledge of Youth Behavioral Issues/Community programs and resources available
Ø NC Driver’s License with an acceptable driving record.
